Long-term Condition Hubs

We are constantly looking at new ways of working in the NHS ensuring patients get teh right care, in the right place, at the right time. In the past few years under the Better Local Care programme, a lot of initiatives have been piloted across Hampshire. Now Fareham and Gosport has been selected by NHS England to collate the best of these findings and accelerate this programme in the local area.

The Willow Group plays a big part in this and since November 2017 a new Complex Care Team has been put in place to support the patients with the most needs, ensuring they will be less likely to require a hospital admission, and more likely to remain independant in their own homes.

As you might be aware we now have Diabetes clinics and COPD clinics at Forton Medical Centre, which is the start of this work.

As part of our on-going work to improve access to routine GP appointments and to build upon the Long-term Condition Hub, we have written to Forton Medical Centre based patients to explain our plans to do this.

We have been working with our Patient Participation group (PPG) on this and there is much more information in the letter below.

We will open Forton Medical Centre on Saturday 21 April from 10-12noon, where you are welcome to come and meet members of the PPG and practice staff to raise any queries you may have.

The surgery is closed for staff training.
