‘SOCIAL PRESCRIBING’ is a service that connects people to resources and services in the community to improve their wellbeing and tackle social isolation.
Supporting people aged 18 and over who have a range of social, emotional, and practice needs, you can be referred by a GP or nurse. You can also self-refer by sending an email to hiowicb-hsi.socprescgosport@nhs.net.
Our Social Prescribers can support you with a host of subjects, issues, and goals, for example
- befriending, counselling, and other support groups
- volunteering, training, and employment opportunities
- volunteering, training, and employment opportunities
- accessing specialist services and support
- opportunities available for education and learning
- healthier and more active lifestyles
Health and Wellbeing Coaches
What is a Health and Wellbeing Coach?
A health and wellbeing coach works in partnership with patients to help them define and move towards their personal health, fitness and lifestyle goals. Coaches strive to build a rapport with patients, encouraging and supporting them to embrace a healthier diet and lifestyle in accordance with their own wishes. Ultimately, we want to empower them to take more control over their own health into the long term.
What can a Health and Wellbeing Coach do?

What issues can the Health and Wellbeing Coach team help with?
We offer up to ten, 30-45 minute sessions to support patients with:
- Weight loss/weight management
- Increasing exercise
- Stopping smoking
- Alcohol reduction
- Menopause
- Pain management
- Prediabetes/diabetes management
- Sleep hygiene
- Prioritising self-care
- Establishing healthy habits
