    To self-refer for maternity care for your pregnancy please follow this link: https://www.porthosp.nhs.uk/departments-and-services/maternity-services/97964 Once you submit the form you will receive an email response giving you information regarding screening and having a healthy pregnancy. A midwife will contact you when you are approximately 9 weeks pregnant to arrange to complete your booking notes.
