Social Prescribing Team

‘SOCIAL PRESCRIBING’ is a service that connects people to resources and services in the community to improve their wellbeing and tackle social isolation.

Supporting people aged 18 and over who have a range of social, emotional, and practice needs, you can be referred by a GP or nurse. You can also self-refer by sending an email to hiowicb-hsi.socprescgosport@nhs.net.

Our Social Prescribers can support you with a host of subjects, issues, and goals, for example

  • befriending, counselling, and other support groups
  • volunteering, training, and employment opportunities
  • volunteering, training, and employment opportunities
  • accessing specialist services and support
  • opportunities available for education and learning
  • healthier and more active lifestyles

Health and Wellbeing Coaches

What is a Health and Wellbeing Coach? 

A health and wellbeing coach works in partnership with patients to help them define and move towards their personal health, fitness and lifestyle goals. Coaches strive to build a rapport with patients, encouraging and supporting them to embrace a healthier diet and lifestyle in accordance with their own wishes. Ultimately, we want to empower them to take more control over their own health into the long term. 

What can a Health and Wellbeing Coach do? 

A Health and Wellbeing Coach  can A Health and Wellbeing Coach cannot 
Help a patient identify what matters to them most about their health Tell a patient what they ‘should’ be focusing on  
Help the patient to identify, set and measure progress towards their own goals Tell a patient what to do/not to do Tell a patient what to eat/what not to eat etc. 
Provide a safe space for patients to open up about their lives, reflect on their health and wellbeing, and identify actions they want to take Provide counselling or mental health support 
Help patients to work through any blocks or practical issues that may be stopping their progress 

What issues can the Health and Wellbeing Coach team help with? 

We offer up to ten, 30-45 minute sessions to support patients with: 

  • Weight loss/weight management 
  • Increasing exercise 
  • Stopping smoking 
  • Alcohol reduction 
  • Menopause 
  • Pain management 
  • Prediabetes/diabetes management 
  • Sleep hygiene 
  • Prioritising self-care 
  • Establishing healthy habits 

As with the Social Prescribers, you can be referred to the Coaches by a GP or nurse, but you can self-refer by sending an email to hiowicb-hsi.willowhwbcoach@nhs.net
