If you live within our catchment area and wish to register with us, you can attend one of our surgeries and ask for a form at Reception. When you bring the completed form(s) back, you will need to provide us with ID (preferably photo) and proof of address.
With the registration form, you will also be given a health questionnaire. The information you provide will be recorded on medical notes.
Newly registered patients are entitled to an appointment with the nurse for a new patient medical. This helps us start our journey together by ensuring we have some information about you and any conditions you might already have. If you currently take medication, please bring it or a repeat order form to the medical so we can enter it on to your notes. If you would like a new patient medical please ask when you hand over your registration documents.
When you register, you will be allocated a Named or Usual GP. This is for admin purposes. You are welcome to make an appointment with any of our doctors at any site. If you would like to know who your Usual GP is, or would like to change your Usual GP, please ask when you next speak to us or send us a secure message through Patient Access.
